When should we have facial paralysis surgery treatment?
Not all cases of facial paralysis (or 7th nerve palsy) are indicated for surgery. Facial paralysis surgery is indicated for the following 2 cases:
- Case 1: When the exact cause of facial paralysis is due to the 7th nerve being severed due to physical damage.
- Case 2: When undergoing other methods such as using drugs, physical therapy, etc., for 3 months to 2 years, the patient shows no signs of recovery.

How long does facial paralysis surgery last?
The duration of facial paralysis surgery depends on what the method is.
For the nerve connecting method, the surgery time is about 2 hours or more. With nerve replacement or nerve transfer, the surgery can last up to 4 hours. As for the muscle transfer method, which is more complicated, the surgery time can take up to about 10 hours.
Facial paralysis surgery can last longer if the patient has previously undergone radiation therapy that caused damage to the blood vessel that needs to be used, and it must be replaced with a longer blood vessel.

Physical therapy after facial paralysis surgery
After facial paralysis surgery, the patient must regularly practice physical therapy to quickly recover the facial muscle function.
For patients undergoing the nerve connecting method, it is necessary for them to practice movement in that area first to restore the movement of facial muscles quickly. For example, if it is the 12th nerve, the patient must practice moving the tongue; and if it is the 11th nerve, the patient must exercise their shoulders.
In order for physical therapy to be effective, during exercise, it is necessary to combine your thoughts and movements to train reflexes to make muscle recover better. At the same time, avoid shoulder/tongue movements that can badly affect the facial muscles.
Recovery of patients after paralysis surgery
If the patient regularly practices physical therapy after surgery, it will take about 2-3 months to recover.
However, the speed of recovery after surgery for 7th nerve palsy depends on the patient’s age, medical history, and the duration of the disease. For elderly patients, the muscles and nerves have aged, the recovery speed will be much slower than for young people (only need about 2 months of hard training).
In addition, people who regularly use stimulants, smoke, or have a corticosteroid disease leading to neurodegeneration will take longer to recover from surgery than people with a healthy body and lifestyle.
